Here's what 2007's order form should look like:

Choose up to 2 different packages in section A and write your desired quantity in the spaces provided. Then add up the sub-totals and write in the total.

Choose the quantity of items in section B which have predifined ammounts listed to the left of the item description to create your own package. After finding your total quantity, the price listing below will show you how much it will cost based on how much your total quantity is.
In Section C enter your quantity desired, and multiply the price by the quantity in the space provided until you are finished and write your total at the bottom of the section.
